Keeping your contact details and other personal information current is part of Maintenance of Certification and exam preparation and helps us deliver important updates to you.
Log in to your Inteleos portal at any time to make updates to your profile.
Changes and exam timing
Any Certificant, Applicant, or Candidate who changes their name or address before taking an examination should take the appropriate steps to update that information promptly to avoid potential issues during Pearson exam check-in.
Important details
For your security, address changes must be updated in your portal account.
Middle name changes can be made in your portal account.
First and last name changes require a name change request and supporting legal documentation.
All documentation submitted must be legible.
How to make a name change:
- Log into your portal account and click on My Profile at the top of the page.
- Under “Update My Profile”, click No under “Is your name shown above correct?”
- Complete the Name Change Request form that appears and upload your required legal documents. You can select multiple files to upload at once.
- If you need to upload additional files after submitting the form, go to the My Resources tab and click Upload Documents. Select Name Change from the File Type drop-down, choose your documents, and click Upload Document.
Prepare all required legal documents for submission:
- Photocopy of a non-expired government-issued photo identification with signature. By law, we cannot accept photocopied or digitized IDs such as U.S. military or certain secure access IDs.
- AND one of these documents:
- Photocopy of marriage certificate, or
- Photocopy of divorce decree, or
- Photocopy of name change decree
Verify change requests via your portal 48 business hours after submission.
Use of contact information
Please note: You cannot unsubscribe from notices concerning your status with ARDMS or APCA, including renewal, CME audit, Knowledge Confirmation (SKILLS quizzes), recertification, or any MOC component.
Third parties
Telephone numbers and/or email addresses are never released to third parties.
Inteleos makes postal lists available for rental for carefully screened third-party offers such as continuing education, recruitment and other industry-related offers.
You may opt-out of receiving promotional mailings via the “Notification Preferences” section of your Inteleos portal.
