Application Letters and Clinical Verification

Some exams require documentation of your clinical experience through an application letter or clinical verification form.

Application Letter Requirements

Some exams require that you provide documentation of clinical experience. Employer- and program-provided letters must:

  • Be printed on official letterhead
  • Include institution/company name, address, and phone number
  • Be dated
  • Include start and end dates of program/employment
  • Specify specialties
  • Signed by hand — stamped or digital signatures will not be accepted
  • Include the ARDMS/APCA number or physician license number in the signature
  • Employer letters only: Specify full- or part-time work and the total number of paid hours
  • Program letters only: Include total program, didactic, and clinical hours
