Some exams require documentation of your clinical experience through an application letter or clinical verification form.

Application Letter Requirements

Some exams require that you provide documentation of clinical experience. Employer- and program-provided letters must:

Be printed on official letterhead
Include institution/company name, address, and phone number
Be dated
Include start and end dates of program/employment
Specify specialties
Signed by hand — stamped or digital signatures will not be accepted
Include the ARDMS/APCA number or physician license number in the signature
Employer letters only: Specify full- or part-time work and the total number of paid hours
Program letters only: Include total program, didactic, and clinical hours
