Meet the training requirements for vascular medicine with one of the following pathways. 

Fellowship Pathway 

Specifications:
Vascular medicine fellowship of at least 12 months duration that meets the requirements in Vascular Medicine within 3 years of application. Those outside of 3 years from their formal training should apply through the practice pathway.

RULE: Physicians enrolled in a 3-year vascular medicine fellowship are required to complete at least 1 year of training before applying for the general vascular medicine examination. In these cases, the fellowship certificate is not required. 

OR 

Cardiology fellowship in an accredited program that includes formal vascular medicine training (COCATS 4, level III) Vascular Medicine 2021 within 3 years of application. Documentation: 

Letter of documentation of program completion (level III) from the program director. 

AND 

Copy of the fellowship certificate. (Not required for individuals following the vascular medicine fellowship RULE as noted in the Specifications section.) 

OR 

Practice Training Pathway 

Specifications: 

Applicant must be a practitioner from general or subspecialty of internal medicine with practice experience of at least 1 year after training (cardiology (COCATS 4, level I or II), nephrology, hematology, etc.).  In addition, the applicant must demonstrate commitment to vascular medicine by dedicating more than 50% of their clinical practice to vascular medicine and/or being active in vascular medicine research. 

The scope of practice should include one or more of the following areas: 

• Vascular medicine clinical consultation 
• Noninvasive vascular laboratory test performance and interpretation 
• Endovascular peripheral intervention 
• Vascular medicine research 

Documentation: 

Applicants must submit a letter signed by the Chief of Staff or Department Chairman. 

This letter must: 

1. Be written on official Practice Letterhead.
2. Confirm that the applicant has dedicated greater than 50% of their practice to vascular medicine and/or bring active in vascular medicine research. The scope of practice should include one or more of the following areas:
   1. Vascular medicine clinical consultation 
   2. Noninvasive vascular laboratory test performance and interpretation 
   3. Endovascular peripheral intervention 
   4. Vascular medicine research 
3. Select the vascular medicine areas that are included in the applicant’s practice (e.g., clinical consultation, non-invasive testing, interventions, or research).
4. Certify that the applicant is in good standing within the medical community.

*Note: Letters must be signed by a physician.  If you are in a private practice/group setting, you cannot have “a partner” write a letter of attestation on your behalf. 

1. Copy of valid, unrestricted license to practice medicine in the jurisdiction of practice. 
2. Copy of current primary board certification (ABIM, AOBIM). 
3. Provide documentation of training requirements for vascular medicine. *Sample Letter* 
4. Fellowship certification if applicable. 

The name on all supporting documentation must be consistent and must match the name under which you apply. If it does not, you will need to submit a name change request with legal supporting documentation (e.g., a copy of marriage/driver’s license)
with your application summary page and all other supporting documentation. Also, Documentation must be in English or accompanied by a notarized translation.

Meet the training requirements for endovascular medicine with one of the following pathways. 

Fellowship Pathway 

Specifications:
Successful completion of a Vascular Surgery, Vascular medicine, Radiology, and/or Cardiology accredited fellowship that included training in peripheral interventional procedures within 3 years of application. Those outside of 3 years from their formal training should apply through the practice pathway. 

Performance of the requisite number of diagnostic (n=100) and therapeutic (n=50) peripheral interventional procedures, at least half as primary operator.

Documentation: 

Letter of attestation of acceptable performance by fellowship program director, which should include a statement that the counting of cases and procedures follows COCATS-4 guidelines as noted above. Also, statement that fellow has acquired the requisite number of diagnostic (n=100) and therapeutic (n=50) peripheral interventional procedures, at least half as primary operator. 

OR 

Practice Training Pathway 

Specifications: 

Have active status in local institution to perform interventional peripheral procedures. 

Be able to demonstrate performance of these procedures within the preceding 12 months. 

Performance of the requisite number of diagnostic (n=100) and therapeutic (n=50) peripheral interventional procedures, at least half as primary operator. These procedures must have been performed within 2 years of the application with 50% within the past 12 months. 

Documentation: 

Letter of attestation by chief of staff or interventional laboratory director from the local institution where active status is maintained. Also, statement the physician has performed the requisite number of diagnostic (n=100) and therapeutic (n=50) peripheral interventional procedures, at least half as primary operator. These procedures must have been performed within 2 years of the application with 50% within the past 12 months. 

1. Copy of valid, unrestricted license to practice medicine in the jurisdiction of practice. 
2. Copy of current primary board certification as noted in specifications section.
